or authorizing your bank (s) to pay by Direct Debit into our
designated bank account.
By Post

Post the completed
Enrolment and
Direct Debit form
to
the address shown on the
reverse of the page or drop off at the nearest Leadway Assurance
Company
Limited Branch.

Please do not send
cheque by post. Arrangements can be made to collect the
premium by Direct Debit from your account (remember to sign
the Direct Debit Mandate).
You may drop the
completed form with Cheque at the nearest Leadway Assurance
Limited Branch.
At the Bank

Please pay the premium at the teller taking care to fill
our account number correctly on
the teller.

Staple a copy of
the teller to your application form and drop it into our

collection boxes located in the banking hall at any First
